Hey Ed. This is called a pivot shoe. I can't be certain without seeing its front, but I suspect this might be our 15-167, which matches the dimensions you list. See if you agree.

I sent a followup picture of the front.Please verify which parts and I will order. the clip on the top which is metal is missing

This does indeed look like the 15-167 to me. Just review the width and thickness dimensions to be certain.
