For the concealed carriage assembly, there does not seem to be a set screw on the pictured assembly. How is this removed? The gear mechanism inside is messed up.

These types of levers are fixed in place once they are installed and would need to be drilled out in order to be replaced. In many cases, the internal mechanism would require disassembly of the door frame. This is not a straightforward process but, unfortunately, it is required to replace this type of hardware.
