Thanks for the additional photos. Our 18-172 Top Sash Guide is not an exact match for the one you have but it is the best replacement option. You will need to drill a new hole into the guides to install them.

In regard to finding the manufacturer, you'll need to search for a sticker that does have identifying information. There is a chance this window may have been made by Kinro but I cannot make any guarantees.
