Tom from SWISCO responded:

Sure, I'm very confident that we can supply this balance. This is either our S380-15 or S385-15, depending on the depth of the bottom nylon fitting. That's the only difference between them. You'll have to measure yours to confirm which one to go with.

Tom from SWISCO responded:

You can order both, if you'd prefer to do it that way. All orders can be placed directly on our site at your convenience. Detailed pricing information is listed on each product's respective store page and shipping cost is calculated automatically based on the delivery option you select during checkout.

A contractor from Osaka. Japan says:

Will you just be able to send me both of them, thus 2 of the S380-15 and 2 of the S385-15

Let me know how to proceed this into a purchase order and let me know how to make payment, shipping etc.

Thank you

Alex

A contractor from Osaka. Japan says:
On the sash weight, is it fair to assume its 4lbs since the old balancer is stamped " 14 4 KQ Caldwell "?

Tom from SWISCO responded:

No, that is not accurate. The stamp is only indicative of a broad calibration, it doesn't represent a specific weight.

Your old stamp of "14 4" matches our more modern 1440. This indicates that a sash can carry 12 through 13 pounds.

You can see a chart on both the S380-15 and S385-15 store page that tells you  how much weight each stamp option supports.
