missing forever door handle

The door is a forever door. the handle was removed by a tenant and lost. The picture shows the lock above and the holes made by the old handle.

Can you tell what handle would fit this?

Hello! We are familiar with the Forever Door, but there are a few options and we want to be certain. Could you let me know how far apart all those screw-holes are? If you tell us the distance between each pair, that's good enough.

storm doorknob screw hole measurements are two inches above and two inches below the center hole
