Thanks for your post! We do not have an exact replacement for the top sash guide you have here. Our 18-171 Stepped Sash Guide could possibly work in place of what you have. It does not have the tab at the top but I do believe it should work for the repair.
Carefully look over the dimensions below and compare them.
