It looks like you have the old Milgard top sash guide. It isn't made anymore but the 18-009 should work as an alternative. Have a look and see what you think. 

Dimensions should be the same but you will note that our top sash guide has a "lip" that sticks out along the top edge which will need to be trimmed. You can do so with a utility knife but make sure to wear gloves just to be safe. Also, note that altered items like this can't be returned for a refund.
