Thanks for posting! I believe you may have the 15-228 Pivot Shoe. This shoe is compatible with either the 1/2" thick Series 160 Coil Balance or the 3/8" thick Series 165 Coil Balance. You'll need to remove your existing shoe and coils to confirm the measurements from the shoe and the thickness of your existing coils. 

You'll also need to look for any stamps/markings on the coils to determine the replacements needed to complete your assemblies. If there aren't any, you'll need to confirm the exact weight of the sash and divide that by the total number of coils in the window.
