Hey Mike. Check out our 15-178 or 15-188 Pivot Shoes. One of these might be the replacements you need, but look over the thickness to see which is the closer match.

Hi Bob
Sorry my shoe is only 7/8 wide. I forgot to include that measurement
Thanks Mike

I believe I can help. Unfortunately, your narrow version pivot shoe has been discontinued. We do have a replacement option, but you'll have to replace the channel balance as well as the shoe. We suggest using the S795-23; not that we've added one inch to your original balance's length to compensate for the old pivot shoe height. You will need to purchase the 15-194 1" T-Lock Shoe, sold separately. You can swap this shoe with the one that comes installed by default with this balance; it's an easy process, as the video below demonstrates. Finally, I believe you need a stamp option of 23B, for a sash weighing between 10 through 16 lbs. You'll also need a new pivot bar I think, so compare your old one to either the 26-021 or 26-241.
