pivot shoe identification

Not sure if its 1/2" or 5/8" thick

Seeing as how your pivot shoe has a black center cam, I think it could be our 15-047 pivot shoe, which is 1-1/4" wide and 5/8" thick. Please make sure these dimensions are the same on yours.
