Hey, Scott! This channel balance looks like it could be either our S380-30 or S385-30, depending on the size of the bottom shoe. That's the only difference between them. Review our dimensions to confirm the better option. All pricing can be found on each part's respective store page.

It also looks like you need a new top sash guide, based on your fourth photo. We can ID a match, but we'll need you to remove an undamaged version and show us a photo of it out of the window and without obstruction. It would be most helpful to see it from the front and side. Think of it like a mug shot.
