S395-21 appears to fit the bill. However the weight stamp is 2090 which does not surprise me as the sash is heavy.

As you can see, the strings attached to the spring are gone. I would purchase everything assembled.

Your identification is correct. However, the 2090 stamp is not used anymore. You'll need to weigh the sash and compare to the option chart on the S395-21 product page.

Thanks Ben! The sash weighs 16# so it appears a 2040 is in order.
