Can you advise a part number and price on these?  I need to replace 18 total.

We don't sell a top sash guide that has the same dimensions you show here. The closest option we sell is our 18-192, but it's still very different. Look it over and see what you think. I also recommend triple checking your dimensions just to be safe.
