sash spring/ balance shoe replacement?

See picture for parts that I need help on identifying to order replacement. The plastic "tabs" broke off on the balance shoe.

Thanks for posting, Laura. This pivot shoe looks like our 15-047. Check your dimensions to see if it's a match. The most important measurements are the width and thickness.
