Thanks for posting! Your patio door handle set appears to be made for an offset latch lever position. That being said, your handle set can be replaced with either the 82-030A Centered Keyed Handle Set or the 82-086 Legacy Keyed Handle Set. 

The non-keyed versions of each of these sets are tagged below.

There are two possible options for replacing your mortise lock; they are the 4-5/8" 82-226 and the 5-3/8"  82-227. To narrow down which of them is the correct fit for your repairs, you'll need to be sure to carefully compare the dimensions for the distance between the installation holes.
