tilt window balance

A quick learner from USA says:
On of my new window decided to destroy itself. I took pictures of everything I thought would help Identify it.

I think I need a pivot bar too

These pictures are a huge help, yes. This is a specific kind of balance that will have to be specially made, though. We would use our Series 770 as the base balance, but then we would swap out the clip at the bottom for our 15-044. Does that look good to you? If so, I'll need just a bit more information.

First, there should be numbers before the stamp pictured. Please let me know what those are. If there are no numbers before this stamp, let me know the weight of the sash instead. Please also let me know how many balances you need altogether.

Now for the other parts. See our 15-216 pivot shoe, if you need it, I'm not sure what the pivot bar might be though; there are a lot of possibilities. Please show me a picture of this pivot bar so that we can identify a good match.
