It appears that the black slider on the hinge assembly is damaged. Unfortunately this part cannot be replaced by itself. The full hinge will need to be replaced. Please start by reviewing the style and dimensions of the 36-048 2-bar hinge set that has a 10" sill track. Note that both the top and bottom hinges will need to be replaced at the same time. Take a look at the video on the page for more details. 

The operator in your last picture resembles a single arm, 39-220B or 39-221B. These operators are compatible with the 39-375 and 39-376 cover and folding handle. Compare the overall style and dimensions to confirm a good fit!

I believe the operator you are showing in your 3rd picture is a different style with a dual arm. If you also need this operator, can you provide additional pictures where we can see a zoomed out view of the arms? Thank you!
