Your operator resembles the 39-217 at first glance. We also offer the 39-216 opposite-handed version. Take a look and see how the specifications compare to yours. Both sides are compatible with the 39-107FK handle with cover kit. Thanks for posting!

What part number is the 11-1/2" guide(3 slots) under window?

It sounds like you may be referring to the track that attaches the operator to the bottom of the sash. If so, see the 39-505 used with these operators. Note that the overall length measures 11-3/8" as shown below.
