There is a identifier on the back of the part C 259 and a stylized Texas with a 10 in the middle.
I heard back from our builder with additional information. The cabinets were made by ACP cabinetry, but they are out of business.

We did carry a bracket like yours at one time but, unfortunately, it has been discontinued and we have not been able to locate an exact replacement. You could consider replacing the slides with something like our 32-056 or the other-sized variants I have listed on this page. These slides work with sockets like our 32-119, which do not require the same 90-degree track end and mount to the back of the cabinet with screws. We can't make any guarantees but take a look and see if you think this setup might work for you.
