I've ordered several of the 39-268 crank operators and they all come with a piece that is not mentioned in these instructions. It's a flat piece of metal with a large hole (a bit bigger around than the crank) and two smaller holes on either side (presumably for screws). What is the purpose of this piece?

That piece is called a trim plate. It is used when the operator is installed behind a wood sill cover trim. When using a wood sill trim, that long operator crank handle will need a hole to come out of in the sill cover. This trim plate will go around the hole to dress it up.

I had pretty much decided that was the probable use (in my case, the crank has to come through a freshly-drilled hole in the screen trim, and this piece looked like it might hide some rough edges). Thanks for the confirmation!
