Crestline weather strip

I need a weather strip for my patio door however I do not know the model of my Crestwood door. The strip is for the side of the door on the right opening door sliding left.Perhaps you have the model on my account as I had a different strip replaced in 2017. The SKU number is 421-3929. Can you help me?

My reference to the door was looking at it from inside the house. There is also another number on my door purchase slip besides the SKU. It is CW SP 2-6068A1. Hope this additional information is helpful to you.

There are two pieces as when I opened the door for the spring season the one piece just fell right off.
