At first glance, this reminds me of our S371-29 channel balance which comes attached with the 15-005 1" deep winged shoe. It's hard to make out if yours has wings or not so I recommend you compare the 15-005's pictures and dimensions to yours to confirm this matches up to your original shoe. If there are any discrepancies, please let me know exactly how right here on this page. 

We also recommend sticking with the stamp that's original to your balance. Choosing a higher spring strength option than what your balance was originally designed for will actually make the window harder to open, not easier.
