Thanks for reaching out to us. The Series 770 appears to be a match. This installs at the top of the window track with a screw but it looks like yours may hook in with a clip of some sort. Can you confirm? Seeing the clip removed from the window and how it installs to the balance would be a huge help. 

Do you need the pivot shoe as well? We may be able to help with that but we'll need to see it removed from the window along with the written out thickness and width dimensions.
