Either the S380-16 or the S385-16 should work to replace the balance you have here. To determine which of these is the correct series, you'll need to measure the depth of the bottom shoe. Be sure to double check that the length of the channel is 16".

Our 1530 stamp should work in place of your 15 3 stamp, but you may want to confirm that the sash weight falls between 10 to 12lbs.
