customer says part fell out of window (probably since old and brittle)
want to replace both sides on 3 windows

This balance is similar to either the S351-21 or the S390-21. Be sure to double check that the length of the channel is 21". To confirm the correct series you'll need to measure the depth of the bottom shoe.

You can likely use the 2020 stamp option but it may be a good idea to double check that the sash weight falls between 8 to 12lbs.
