It looks like you have either our Series 351, with a 15-002 bottom attachment, or Series 390, with a 15-001 bottom attachment. Take a look at the image below to determine which balance you'll need. It appears that the bottom shoe of you balance is broken. You will need to measure an undamaged version to determine which series you have.
Once the length is determined, take a look at your balance for a number stamped into the metal channel. This number will determine the spring strength stamp option needed to support the window sash. If there is no number present, you will need to obtain the weight of the window sash and compare to the strength chart on the product page to choose the correct option.
