These are failing throughout my house. This one is 36 3/4 in long.  There is a trademark in the close up

I can't be sure with the balance still installed but the S795 would be a good place to start. You'll need to remove your assembly to confirm if the 15-195 T-lock shoe that comes with these balances matches youse, as well as the screw clip at the end of the pulley. I'd also recommend double-checking the length, which would normally be 26" based on the 26C stamp shown in your second photo. Thank you!
