Looks like you need a new pivot shoe. We sell a ton of different parts that may replace this, but I'm not sure what to recommend without knowing size.

For now, start by looking at our 15-046. Carefully review our dimensions and see how well they compare to yours. If they differ in any way, let me know how and we can work from there.
