Your balance resembles our S765 channel balance but the top of the balance is not visible, so you'll need to confirm if the pulley clip that extends from the top of the balance matches yours. You'd also need the stamp code printed on the metal channel to choose the corresponding strength option.

As for the pivot shoe, the S765 is commonly used with the 15-046 but it measures 1-1/4" wide as shown below. Take a look and see if this could work for your repair. Thank you!
