Thanks for your post! This balance is similar to a few different series we supply. To narrow down the correct replacement, I'll need to see a photo of the spring side of the balance. I'm particularly looking to see the bracket connecting the spring and the cord (string), in the middle of the balance.

Thanks for the additional photos! This window balance can be replaced with either the S380-21 or the S385-21. You'll need to measure the shoe's depth to determine the correct balance replacement.
