21" channel balance

A handy person from Chino, CA says:

There is no label or manufacturer info on the window itself. The part that is attached to the window that hooks to the upper nylon guide (damaged) came loose which caused the issue. The sash weighs 13#.

I have selected 2 of the S385-21 & option 2030 which I think are the correct replacement.

Hello! I agree, our S385-21 looks like a good pick. The only other possibility is our S380-21; this is identical to the S385-21 except the shoe depth is a little smaller. Review the diagram below to see what I mean.

You also mentioned a damaged nylon top piece. That sounds like a top sash guide, though I don't immediately recognize the one in your photos. Would it be possible to see a closer photo of it? Seeing it from the front and side would be the most helpful. Think of it like a mugshot.
