Start by looking at our S385-28. This comes with the 19-005 top and 15-004 bottom, so it's important to make sure these pieces match what you have now. I would also recommend going with the 2740 option for the S385-28.

I purchased the recommended S385-28 - option 2740 and they worked perfectly. After installing, I realized the top sash guides are broken but I can't locate a match on your site.

Great, glad to hear our balance worked out! As for this top sash guide, this is an older part no longer made. Many customers have gotten our 18-171 to work; this is more or less the same, except the top cap is much longer. If it cannot install as-is, you can cut the top down.
