Needing replacement balances (right & left) for upper sash of double hung Silver Line window, please. Sticker on window shows: Silver Line 80023442288, Product ID 3001, Actual Size 47 3/4 x 72.
I am also needing to purchase replacement Upper Sash Pivot Bars. The measurements on mine match your Product Code 26-400. However I would like to order the part that includes the plastic piece. Would this be Product Code 26-427?

Your balance appears to be a match for the Series 795. Your old T-lock shoe matches the 15-195B (sold separately) with an open cam, so you'll have to swap the default shoe, as shown in the video below. Note that you must measure only the metal channel to determine the length of the replacements. We'll also need you to confirm the precise weight of the window sash since your stamp doesn't match our options.
We can't confirm is the mentioned parts are what you need for your old pivot bar, however. We would have to see it removed from the window sash, along with the plastic portion.
