Thank you for reaching out to us. Take a look at the 15-177, which appears to match what you have here. The Series 152 (sold separately) is designed to screw to the side of the pivot shoe. 

We can't be sure of the pivot bar since it's damaged, but you can start by taking a look at the 26-226. You'll want to compare the specifications with an undamaged version to see if they match your repair needs.
