Thanks for posting! We do not have a direct replaement for this shoe but we've frequently recommended the 15-191 to replace it in the past. Take a look over the specifications and compare them to confirm this will be a fit.

If you are in need of a replacement for the pivot bar, I'll need to see a photo of an undamaged one removed from the bottom of the sash.

This shoe is typically used with the 1/2" wide Series 160 Coil Balance, but you'll need to confirm the width of your coil to confirm this is the right series. If so, you'll need the S160-04, which is used with the 15-150S Coil Single Support.
