Several balances are broken per the photos. There is no weight number on them. I also need the pins. The screw hole for that is 2 inches from the end of the pin per the photo. These are screwed to the frame through the middle of the spring. I estimate the sash to weight 15 lbs. I am very handy and can handle the swap out.

Thanks for posting. This looks like a 15-180 Pivot Shoe, along with the 15-186 Balance Clip and 15-182 Bushing. This system works with Series 190 Coil Balances. If the window weighs exactly 15 pounds, no more, then you could use a pair of the S190-740 7 pound springs, for an overall weight support of 14 pounds. Being within 1 pound of the exact weight works just fine. We highly, highly recommend you weigh the sash on a scale to confirm the precise weight before proceeding. For a fastener, you might be able to use something like the 33-073, though this can depend on the window track itself. Take a look and let us know if you have questions.

Thanks for the detailed reply. Which pivot bar should I use based on the single screw hole being 2" from the exposed end of the bar?

It might be the 26-040, which is often used with the 15-180, but you'll have to compare your old one to the diagram below to confirm.
