Thanks for writing in! Your window balance can be replaced with the Series 795 Channel. Based on the stamp, you'll need the 22" S795-22 with a 22C stamp. 

The shoe on the bottom is a 15-194 1" T-Lock Shoe. This is sold separately from the balance. You'll need to swap it out with the shoe that comes installed on the balance.
