This looks like the older version of our 1" wide 15-194 T-lock balance shoe. However I'm not sure if it can work on your balance.
See our Series 795 channel balance. I suspect this is what you have, though it comes with our 1-1/4" shoe. You would need to swap the original shoe out with our 15-194.
