Any help identifying this make/model and or replacement parts would be appreciated. I'll be needing some other minor parts as well

Thanks for your post! The coil assembly you have here appears to be an older, discontinued Simonton balance. To replace it, we suggest using the 15-259 Coil Balance Pivot Shoe along with the Series 159 Coil Balance; the dimensions for it can be found directly on the website. Take a look at both of the video tutorials below as a reference for the information you'll need to compare and gather to cover this assembly.
