The compatible balance for this shoe is the Series 152 Coil Balance secured to the shoe using the 33-072 screw. To determine exactly which coils are needed for your assembly, you'll need to look for any stamps/markings. If there aren't any, you'll need to obtain the exact weight of your sash and divide it by the total number of coils in the window; this will give your the number of pounds you will need for each coil.

If you are looking to replace the cover, you'll need the 15-143 Coil Bushing, the 15-144 Coil Bushing Cap, and the 15-145 Coil Support Barrel.
