Thanks for reaching out to us. I recommend our Series 795 for this repair. There should be a stamp printed on the channel as well. You'll also have to purchase the 15-194 shoe separately and swap it with the default one it comes with. It's very easy to do so. The video below will show you precisely how. 

The Series 795 installs with a screw but if yours uses a hook, please let us know! We can still help though this would have to be a custom order.
