Your balance resembles the Series 770 channel balance, but with the 62-105 hook attached instead of the terminal clip. This hook attaches to the 15-105 pivot shoe which yours resembles. We can provide this balance via a custom order. 

Please review the dimensions of the pivot shoe to confirm a good fit!
