Block and Tackle balance identification

I need to replace a broken balance for my single hung window. The pictures show the broken balance. The length of the metal channel is 21 inches and width is 5/8 inches at the widest. The weight of the sash is 21 pounds. As the pics show, the top and bottom guides are both winged. I'm looking for advice on the right the Swisco parts to buy for my replacement balance.
