Broken Parts not Window won't slide up and down the track properly

A quick learner from Ohio says:
Broken Guide Bolts from inside the window itself that prevent the window from coming out of the frame broke, and the part inside the frame that the bolts rest into are broke. These are double hung windows, I don't know who manufactured them, however I included a picture of the warranty that was located in the top of the window frame but I can't relate it to anyone.

Hello there!

Thank you for reaching out to us. I suspect you may have the 15-177 pivot shoe which is designed to use the S152 coil springs. This part measures 1-1/4" wide and 9/16" thick so I recommend you remove your old pivot shoe from the window track to confirm the specifications align.

I can't be certain since your pivot bar is broken but our 26-226 comes to mind. Take a look and see how the dimensions compare with an undamaged version.
