Broken rivet and pieces on channel balance

Question

Hi, the rivet where the spring attached failed and this assembly doesn't work. Channel length is 33" and plain. Window is tilt-in. I think the black piece at the top of the channel, the pulley piece with the string around it, and T at the bottom may all have sustained some damage. Can you help identify the replacement parts I need to order? I also included a pic of the warranty sticker on the side of the to help identify the window this assembly is from. Thank you!

Your balance appears to be a match for our Series 795. This would be my top recommendation. Please note that this installs at the top using a screw, not a hook. If your balance uses a hook then I need to see a photo of the hole in the frame where it installs before we do anything else.
