Broken sash pivot?

Hello. The right side sash pivot broke on one of my windows, and I'm trying to see if I can get a new one and instructions on how to install it. I've included a photo of the broken part and I found the label with the window information, and that information is - 
Serial Number is 213-533353
The P/O Number is 9461
The Date of Manufacture is 5/14/2013
The Window Size is 30 3/4 x 53
