Hey there. This looks like the 15-087 Pivot Shoe and 15-086 Tandem Housing. This system works with Series 180 Coil Balance springs. The '4' and '5' likely indicate you can use the S180-04 4 pound spring and S180-05 5 pound spring; you can weigh the sash to confirm this is right if need be. See below.
