Your casement lock looks like an older version of our 38-097 and 38-098. See if you agree. If so, keep in mind you may also need a new keeper. See our 38-079.

As for the operator, take a look at the 39-014 below. Review the dimensions and style of the disc that rides in the sash track to see if this matches your original assembly. Thanks for posting!
