We'd be happy to look into this for you but we'll need a bit more information to narrow down your options. If you could show us the connecting point between the spring and pulley inside the metal channel, that would help us confirm if our stock balances could work for your repair or if they would need to be special ordered.
As for the operator, take a look and compare the 39-217 shown below to your assembly.
Unfortunately, we do not carry an exact match for your sash lock but the 90-323 is similar. We can't guarantee it would work since the style is a bit different but it would be our best option. Review the specifications to see if this style could fit your window. Thank you!
Our closest stock balances appear to be the S380-21 or S385-21 (2040) depending on the depth of your bottom fitting. With that being said, our stock balances will not work if the spring and pulley system is another style commonly used by Caldwell. This is why we are requesting an additional photo showing inside of the balance spring and pulley mechanism.
The opposite-handed version of the operator shown in your photo would be the 39-216 shown below.
Unfortunately, we are not able to confirm exact measurements from photos but it does look like your shoe is over 1" deep. If so, the S385 would be the way to go since your balance has the same spring and pulley connection as our stock balances.
