Printed on it reads 26 4 AS CALDWELL
I found one video to identify the top and bottom. Top is 15p142. Can't tell what the bottom is because its worn away some.current measurement of the bottom is just shy of 1 inch

I also have a broken removal clip, photo attached

Based on your dimensions I think you will need our S380-27" channel balance with spring strength option 2640. Use our 16-002 to replace your take-out clip.
